NO: 15-5-00233-0 NOTICE OF PETITION TO TERMINATE PARENT-CHILD RELATIONSHIP IN THE SUPERIOR COURT OF THE STATE OF WASHINGTON IN AND FOR THE COUNTY OF PIERCE In Re the Interest of: SPENCER THOMAS RICK, A Person Under The Age of Eighteen TO: JOHN DOE, to include any unnamed natural father. YOU ARE HEREBY NOTIFIED that there has been filed in this Court a Petition to Terminate the Parent-Child Relationship between you and SPENCER THOMAS RICK, a male, whose natural mother is JESSICA NICOLE NEAL. The child was born on May 19, 2010 at Good Samaritan Hospital, Puyallup, Pierce County, Washington. The Petition prays that the Parent-Child Relationship between you as natural father and SPENCER THOMAS RICK be terminated. A hearing on said Petition will be held on the 1st day of May, 2015 at 9:00 a.m., before a Judge or Commissioner of the above-entitled Court, at Remann Hall, 5501 6th Ave., Tacoma, WA 98406, at which time and place you and all interested parties shall appear and show cause why the prayer of the Petition should not be granted. YOU ARE NOTIFIED that any nonconsenting parent or alleged natural father has a right to be represented by counsel and that counsel will be appointed for an indigent person who requests counsel. YOU ARE ALSO NOTIFIED that you have a right to respond to the petition, within thirty (30) days of the date of service of this Notice. Failure to respond to the termination action within thirty (30) days of the date of the first publication of the notice is grounds to terminate your parent-child relationship with respect to the child. You may respond by filing a claim of paternity under RCW 26.26 or filing as a direct responsive pleading hereto a Notice of Intent to Claim Paternity. As a result of your filing of a claim of paternity pursuant to RCW 26.26, you may be entitled to certain presumptions under RCW 26.26.040, if applicable; to blood tests; DNA tests; to a trial on the issue; to present evidence and rebut evidence against you; and to an order determining your paternity or nonpaternity.
