Cook failed to appear on July 26 in Pierce County for a pretrial hearing on charges of filing a false insurance claim, first-degree attempted theft and failure to appear in court. He was charged in July 2015 and previously failed to appear in court in February and June 2016.

According to the investigation, Cook was involved in an auto collision in August 2013 with a truck owned by a plumbing business. The driver of the commercial vehicle ran a red light and Cook, who had the right of way, collided with him. According to the investigation, Cook filed a wage-loss claim with the plumbing company’s insurer using falsified documents. The insurer and SIU detectives could not find evidence that the company Cook said he worked for existed.

Cook also failed to appear in King County Superior Court in a separate insurance fraud case. He was arrested the next day when he appeared in King County Superior Court in an unrelated case.
