TO: CHRISTOPHER FERGELIC, nonconsenting father: You are named as the father of the above-named children. A Petition for Termination of Parent-Child Relationship has been filed in the above-entitled court. The Petitioners are asking the Court for an order permanently terminating the parent-child relationship between you and these children, permanently terminating all of your rights to the children and for a Decree of Adoption declaring the Petitioners to be the legal parents of the children. CHRISTOPHER McDONALD was born on the 18th day of November, 2009, at Puyallup, Pierce County, State of Washington at the Good Samaritan Hospital. His mother is Meagan Conwell. Her name was Meagan Fergelic at the time the child was born. GRACE McDONALD was born on the 24th day of April, 2012 at Puyallup, Pierce County, State of Washington at the Good Samaritan Hospital. Her mother is Meagan Conwell. Grace McDonald has no father listed on her birth certificate but it is presumed that Christopher Fergelic is the biological father. In order to defend against this Petition, you must respond to the Petition by stating your defenses in writing and by serving a copy upon the attorney at the address below within twenty (20) days after the service of this Summons, excluding the day of service, or within thirty (30) days (if service is made upon you outside the State of Washington), or a default order permanently terminating your parent-child relationship with these children will be entered by default. A default order is one where the Petitioners are entitled to what they ask for because you have not responded. If you serve a notice of appearance on the attorney at the address indicated below, you are entitled to notice before a default order may be entered. You have the right to be represented by an attorney. If you are determined indigent and request an attorney, an attorney will be appointed for you. Your failure to respond to the Petition for Termination of Parent-Child Relationship filed herein is grounds to terminate your parent-child relationship with respect to the children identified above. If the children are Indian children as defined by the Indian Child Welfare Act of 1978 25 U.S.C. 1901 et Seq., your parental rights may not be terminated unless you give valid consent to termination, or your parent-child relationship is terminated involuntarily pursuant to chapter 26.33 or 13.34 RCW.
